Documenting A Client

Demonstrate the importance of documentation and best practices.

If it isn’t documented, it didn’t happen. That is the most important thing about documentation. As a case manager or counselor, it is your responsibility to keep a record of all contacts relevant to the case/client. This chapter will help you understand how to type a contact and/or meeting notes and the importance of good documentation.

• Read Chapter 23 in your textbook.
• Post your response in the discussion forum to this scenario:
  • Using the example of meeting with a client at the bottom of p. 405 and continuing to p. 409, write up a case note summarizing your meeting.
