SJSU Companys Carbon Steel Pipeline Has Thinned out Due to Sulfidation Corrosion Memo

It is August 1, 2012, and you are an engineer working at the Chevron Oil Refinery in Richmond, California. Karen Henry, an EPA Emergency Management inspector, has recently contacted your division requesting a copy of your Earthquake Emergency Response plan. Your supervisor has charged you with writing the new safety procedures. Your research uncovers an inspection report from the previous year that includes the following recommendation: “section of a carbon steel pipe, installed in 1976, has thinned from sulfidation corrosion to the point of failure and should be immediately replaced.” You search the file for a follow up report or memorandum indicating the pipe had been since replaced. There is no further information in the file. You immediately call your supervisor and report the finding, but the supervisor tells you to work on the Earthquake policy and not to worry about the piping. After a site visit and careful inspection of the piping, you can see that the replacement was never done. What’s the right thing to do?
