Open and clear communication plays an integral role among staff from different level of the treatment pyramid. Different level of staff such as nurses, pharmacists, Doctors and social workers among others actors are like well-oiled machines to limit adverse events (AE), improves outcome, by maximizing patient safety, decrease patients length of stay (LOS) and adds to patient and employee satisfaction. (Epstein, 2014). Due to the broad nature of the multidisciplinary team in the management of complex illnesses, it is essential for specialties to clearly and effectively communicate with one another to foster collaboration, teamwork, medical error identification and preventions. A key factor in achieving high quality of patient care is collaboration among health professionals of various disciplines. (Vos, et al, 2020).One of such tools that enhance coordination care and support team work is electronic heath record. (EHR).
