I will attach the reentry case plan.. you are basically writing a one page summary on the plan.

Your summary is simply a written overview of your client; their needs, things that concern you as their reentry coordinator, etc. This should only be about one page in length. For example;

“Marcus has spent 18 years in a state penitentiary. He has extended family that he can return to but many of them are gang affiliated and that is certainly a concern. He is dedicated to work his reentry plan but we need to be mindful of this connection. Because of this, I think it best that I meet with Marcus twice a week for the fist 3 months post-release.

Extremely dysfunctional family; no ties to them

Sexually violated from the age of 9-16 by step-father

She was a runaway at the age of 16 and became prostitute to survive

She is a neutralizer; condemnation of the condemners
