Proposed Agency Project-5 Slides

Each Learning Community Group (LCG) will be responsible for developing a proposed health or human service program designed to meet an organizational or community need. Each LCG will prepare a total of three items for their proposed agency or service.

1. Proposed Agency Brochure

The program brochure must be professional in nature and clearly outline the agency name, mission and vision, service(s) provided and fee structure, service location(s), contact information, and days/hours of operation of the proposed agency or program. Students must obtain written permission to use logos from existing agencies and/or organizations.

2. Proposed Agency Website

The website should be an extension of the brochure. The website must clearly outline (1)agency name, (2) mission and vision, (3) service(s) provided and fee structure, (4) about us section with organizational chart, (5) Key Staff bios and job description, (6) resource/referral page with a minimum of 5 working links, (7) service location(s), (8) contact us information, and (9) days/hoursof operation. Students must obtain and submit written permission to use logos from existing agencies and/or organizations.

3.Proposed Agency PPT

The PPT proposal should provide a detailed overview of the program and must include (1) description the need and rationale for the program. (2) value and benefit(s) in developing and program with client/constituent population, (3) theory of change and explanation of how and why the proposed service components are expected to change or enhance client or constituent functioning, (4) clear and detailed discussion of program services and components, (5) comprehensive evaluation for all program service components, (6) review of relevant scholarly and professional literature (approximately 8 to 12 articles) that validate and support the proposed program services/components, and (7) a logic model of the proposed program.
