Introduction: Performance measure selected from the domain is Effectiveness of Care under Cardiovascular Condition: Controlling Blood Pressure.

1. Self Blood Pressure (BP) monitoring at home, you must include how to measure the outcomes. 

2. Healthy dietary intake (diets low in salt ), you must include how to measure the outcomes. 

3. Physical activity and exercise routines, you must include how to measure the outcomes.
