>> So, Victor how’s everything going so far?
>> Great! Yeah, everyone’s really nice and my system everything works pretty much the way I learned in school.
>> Excellent. Excellent. So, what’s up?
>> Well, I know we’re going to be transitioning over to ICD10CN and ICD10PCS and I’m just wondering if there’s anything I should be doing to prepare for that
>> No. Don’t even worry about it. We don’t even have a deadline set up yet for the changeover and even when that happens all that’s really going to entail is the vendor coming in and updating the system and that’s really their problem not ours.
>> Okay.
>> Yeah, I mean, they’ll probably do one day training for all the coders but that’s really going to be about it.
>> Okay.
>> Anything else?
>> No. That was it.
>> Okay. Okay. Excellent. Then I’ll talk to you later then.
>> Okay. Definitely isn’t what we learned in school.
>> Victor Diaz, [assumed spelling] the coder, came to see me. He said that he knew we’d be transitioning to ICD10CM and ICD10PCS and he wanted to know what sort of plan we had in place to manage the transition. It was odd because the natural thing for him to do would have been to ask Vanessa Wiley, [assumed spelling] his coding supervisor but, for whatever reason, he found Vanessa’s answer less than satisfactory. Well he knew from his classes that it was a pretty extensive process. Here’s what I told him. I said that I’d put together an implementation team to run the transition and make sure nothing falls between the cracks. I said that the changeover is going to affect information systems, HIM, and administration, so we’ll have to be familiarized. The whole clinical staff will have to be trained on the documentation requirements so that they’ll be able to assign the proper codes. Some of all this is going to be done by us and some we’ll do with the help of the vendor. Also, they’re going to test the new system after it’s in place so at least we won’t have to worry about that. Victor wanted to know what he specifically could do to get ready for the changeover. I told him that he should start by reading articles in HIM journals about ICD10CM and ICD10PCS and to continue visiting the CMS website for up to date information. He asked good questions and he got me thinking that I should probably put out a memo and detail all this for the coding staff. Not everyone is going to be as proactive as Victor was in preparing for the transition but I should at least let everyone know what to expect and how best to prepare for it.

>> I was, yeah.
>> Well, have a seat. Okay, order the ones that are off premise and give me a call when they get in. Okay, thank you. To what do I owe this visit?
>> Hoping you can help me with a new research project.
>> Oh possibly. What kind of research are we talking about?
>> Well, we have a new experimental drug that’s in the next stage of testing. We’re looking at efficacy and outcomes, of course. And I’d like to get the HAM department’s help in planning and coordinating. You know, collecting data on potential candidates that we can approach to participate in the trial.
>> Well, we can certainly help you with that. We’ve done quite a number of those already this year.
>> I knew you were the go-to person for this. I’d like to schedule a meeting fairly soon so that I can present the specifics to you and to any other personnel who may need to be involved.
>> Yes, well, that would be the next step. Why don’t you have your office contact me and we can go over the information needs for your clinical research trial.
>> How about next week?
>> Sure, that sounds good.
>> Excellent. Hey, I’ll have Rob at my office give you a call.
>> Cool.
>> And I’ll see you next week.
>> All right, I’ll see you then.

>> Once a research project has been established and a hypothesis has been developed, a data collection tool can be established. Now data can be collected one of two ways. Either manually or electronically. Statistical software is helpful in both gathering and analyzing the data. The HIM staff may write an ad hoc report to help swing potential candidates for the research based on various criteria. Some of those criteria may include selection by a particular diagnosis, a procedure code within the coding classification system, ethnicity, age, gender, or blood type. Whatever the screening parameters, they must be reviewed in advance with the physician conducting the research. Address the HIPAA privacy rule to assure compliance concerning choosing the data collection process. Once the data is collected, HIM can help by analyzing the results, determining the outcome, and preparing and presenting the findings using tables, graphs, or other visual aids.
