When you hear the term transformative healthcare policy formulation, what does it mean to you now that you have read about it? Provide an example.

Students – according to the CDC (2017) the American healthcare system is in the midst of unprecedented change. The U.S. health care and public health systems are both now positioned to place greater emphasis on better care, smarter spending, and healthier people. Key elements emerging in this transformation include new structures for integrating and coordinating services, a renewed focus on patient engagement and patient-centered care, and new payment models based on the value of population-based health outcomes rather than the volume of services delivered. This period of change is creating important opportunities to establish effective, more sustainable models to improve population health (para. 3). What are your thoughts on these areas of improving and transforming population health?

Week 4 Assignment: Public Health Policy Paper

Objective: Create an organizational policy to lessen the outcomes of infectious disease exposures on human health.

After reading the chapters assigned this week, access the website page of the CDC Current Outbreak List. Choose and click a current outbreak from the list and create a policy that an institution could adopt to prepare for and combat the outbreak.

Imagine that you are the healthcare administrator for a 100-bed facility.

Addresses the following in your policy:

• Name the outbreak and include when and where the outbreak began.
• Access the Map of Reported Cases or the Outbreak Area and include the geographical information.
• List the symptoms. Discuss the prevention options.
• Describe some policy elements that would help reduce the effects of the outbreak at your facility.
• As a healthcare administrator, how could outbreaks affect your facility?

Your paper should be two to three pages. Include a title page and a reference list. You must cite and reference the website and at least one other source. You may use your text as a source.
